All forms must be turned in on the first practice date. Failure to do this will exclude the athlete from participating in team activities until forms are completed.

• Every student athlete must have an Emergency Medical Authorization (EMA) form on file with the athletics department by first day of practice. 
  
• For you convenience University Suburban Sports Medicine Center will be offering Pre-Participation Sports Physicals on Saturday July 19, 2008.   A physical from you family physicians will also be accepted.
