Overnight and Extended Field Trip Medication Information
Your daughter will have the opportunity several times during her Laurel education to attend overnight or extended field trips. These are the class trips/club trips not associated with the Passport Programs. Passport Programs please refer to Beyond the Classroom on the Laurel website.
In accordance with the Ohio Revised code, the following guidelines must be followed for the administration of student prescription, over the counter and non-prescription medication at Laurel School. Please refer to the Medication at School link on Nurse’s Office web page.
PRESCRIPTION MEDICATION:
- Prescription Medication Authorization form needs to be completed then signed by the PHYSICIAN and PARENT. (Form can be printed from NetCommnity on Laurel website)
- This is for ALL PRESCRIPTION Medication there is NO exception.
- This procedure is required by Ohio law and must be followed.
- This form may be completed and faxed to the physician for signature and then faxed to Laurel at 216.464.8996 or you can turn in a printed copy yourself.
- Send only the amount of medication necessary for days of the trip in a pharmacy labeled bottle. Your pharmacy will provide a properly labeled bottle to match the prescription. Medicine must be in pharmacy container and must match the written prescription directions.
- NO baggies or self-labeled bottles accepted
OVER THE COUNTER MEDICATIONS (OTC):
As within the guidelines of the school medication policy- Laurel will provide the following over the counter (OTC) medicines. You should have completed this form in NetCommunity at the beginning of the school year giving permission for the following medications.
- Acetaminophen (generic Tylenol)
- Children’s Tylenol
- Ibuprofen (generic Advil, Motrin)
- Children’s Advil
- Benadryl
- Pseudoephidrine HCl (generic Sudafed)
- Robutussin DM
- Cepacol lozenges
- Sunscreen: Sun-X, SPF 30, PABA free, oil free, UVA&UVB protection
Contains aloe vera gel, vitamin E and cedar fragrance - Insect repellant: Off! Skintastic for Kids with Aloe, unscented, DEET-4.75-7.0%
OTHER MEDICATIONS: If you are giving your child any OTC medication NOT LISTED on this form for example: Pamprin, Halls, Dramamine, Tums, etc. please do the following.
- Complete the Non-Prescription Medication Form. (Form can be printed from NetCommunity at the Laurel website)
- No physician signature is required.
- Medication needs to be sent in its original unopened packaging.
- Suggestion: purchase the individual size medications.
The forms and medications must be received by the specific trip due date or they will NOT be given.
